*EXECUTION OF THIS DOCUMENT IS A CONDITION OF AMENITY USE UNTIL FURTHER
NOTICE*

In consideration for the ability to access and use the recreational amenities of Vibe at Liberty Village (“Amenities”) during the current COVID-19 pandemic and related restrictions and guidelines, I, the undersigned, on behalf of myself, my family and dependents, hereby acknowledge, understand and agree to the following:

1) I understand the risks of COVID-19 and have familiarized myself with the guidelines
provided by all levels of government. I further understand that circumstances
regarding COVID-19 can change on a daily basis and, accordingly, guidelines are
subject to modification.

2) I acknowledge, understand and agree that notwithstanding any steps taken by the
Corporation to reduce the potential risk of spreading COVID-19, the Corporation
cannot eliminate the risk and therefore there is a risk of being exposed to and
becoming infected with COVID-19 as a result of my use of the Amenities, which is at
my sole risk.

3) I freely and voluntarily choose to make use of the Amenities regardless of the risks
associated with COVID-19.

4) I will fully comply with all guidelines and conditions for use of the Amenities as the
board of directors or property manager may require from time to time.

5) I acknowledge and assume the risk of illness or death related to COVID-19 arising
from my use of the Amenities and hereby release and discharge Vibe at Liberty Village TSCC 2119, 1817 & 1977 and its officers, directors, agents, employees, contracted staff, unit owners, residents, assigns and insurers (the “Releasees”) from any liability related to COVID-19 which may occur
as a result of my use of the Amenities.

I further represent and warrant that, with each and every use of any Amenities, I:

1) Am not currently experiencing COVID-19 symptoms nor have had any within the past 14
days. If I do develop such symptoms, I will advise the condominium manager of this
immediately.

2) Have not travelled outside of Canada or been in an area under a travel health advisory
within the past 14 days.

3) Have not provided care to or had close contact with any person with COVID-19,
reasonably suspected of having COVID-19, or who has travelled outside of Canada or
been in an area under a travel health advisory within the past 14 days.

4) Have not been advised by any level of government or its agent to self-isolate due to
possible exposure to COVID-19.

5) Am over the age of 18 and a current resident of Vibe at Liberty Village and not a guest, visitor, or non-resident unit owner.

I understand that there are inherent and unforeseen risks involved in the use of the
Amenities. I have read this document in its entirety and fully understand its terms. I
understand that I, on behalf of myself, my family and dependents, am giving up
substantial legal rights by agreeing below, including the right to sue the Releases.

UNIT ACCESS AUTHORIZATION/WAIVER

Please be advised that TSCC 2119 has reviewed the process of access being provided to the building and usage of the master key for granting unit access to Registered Owners, and/or Registered Occupants (as those terms are defined in TSCC 2119’s Unit Access Policy).

This process requires that all Registered Owners and Registered Occupants of a unit duly complete, execute and submit this authorization and waiver form, which will permit TSCC 2119’s agents to grant access to Registered Owners and/or Registered Occupants, to the building and to the unit, if applicable.
If a completed copy of this form is not on file for your unit, then we will not provide any Registered Owner and/or Registered Occupant, access to the building or to your unit.
Please complete and execute this form and return it to TSCC 2119’s concierge or the Management Office. Please note that it is your responsibility to ensure that any information provided to TSCC 2119 (whether it be on this form, or otherwise) is accurate and updated accordingly. Any changes to the information on this form must be brought to TSCC 2119’s attention immediately.

I, the undersigned, a Registered Owner and/or a Registered Occupant of a unit in TSCC 2119, give my authorization for TSCC 2119’s agents to permit the following Registered Owner(s) and/or Registered Occupant(s) access to the building and to my unit. I acknowledge and agree that once this form is properly executed and submitted, TSCC 2119 and/or its agents are under no obligation to contact the Registered Owners or Registered Occupants prior to permitting access to the building and/or to my unit, as authorized herein.

I release, forever discharge, and agree not to hold TSCC 2119, TSCC 2119’s agents, management, staff, or any related parties, responsible for any issue, damages, loss, injury, or claim that may arise from TSCC 2119 or any of its agents granting the following Registered Owner(s) and/or Registered Occupant(s), access to the building or to my unit.
